Coaches / Team Managers,
In order to request an approval for a Tournament it is imperative each coach complete the appropriate form(s) and follow the directions below:
1. Fill out the "Tournament Check Request Form", print out a copy of the Tournament Invitation & Referee Team Page along with one check for the complete fees Payable to "AYSO Region 117".
2. Submit the forms to Nikki Kakuuchi at 9114 Adams Ave #150, Huntington Beach, CA 92646 or via email at [email protected] .
3. The forms and payment will be processed and a Tournament Roster will be approved.
4. Payment will be sent once all approvals are met.
5. Once approvals are done a Roster will be provided.
After the completion of the tournament, you will need to fill out and submit the "Referee Deposit Refund Request Form" in order for you to get your referee deposit back. Once the region has been reimbursed by the tournament, then, the region will issue you a check.
